Below you will find all registration forms for the 2025-2026 school year. Please review the program information sheet prior to submitting registration forms. Rates vary by program. To view all available rates, please select the forms based on the age of your child. These forms show all schedules and their associated rates for the 2025-2026 school year. To register, please complete the appropriate form and return to the school with your registration fee.
Monday, January 27, 2025: Registration opens to currently enrolled families. Current families can send forms to MCDS in their child’s bag.
Sunday, February 09, 2025: Registration opens to the public. New families can drop forms off at MCDS from 1:00 PM – 3:00 PM.
